New Patients

How to Register as a Patient

We wish to extend a warm welcome to new patients joining the practice. 

To register, you will need to fill out an NHS form and the Cowplain Family Practice Registration Form which you can download here.

When you have completed these forms, please return them to reception, where you will be asked to present your two forms of identification, one being photographic. Once your application has been accepted, we will ask you for your preferred doctor and then do the rest for you. 

If you are over 15 years old, we will ask you to make an appointment for a new patient health check with one of our team.
